Davis county residents who are at or below 185% poverty level. Income restriction does not apply for the first 3 visits. Help is also available on a case-by-case basis for those who may experience a temporary setback or find themselves in unexpected circumstances.

Walk in. After 3 visits, a registration process will occur with income review.

Photo ID for adults over 18, ID for each child (Birth certificate/passport, or Medicaid card or document, or current school record, or any state/federal document that includes name and age.) Verification of address (rent or lease agreement, utility bill, current mortgage or tax statement, or official recent letter from bank, state or federal government;) Verification of income (pay stub, or Social Security or Disability payment information, child support/alimony agreement) showing monthly earnings.
