Application Process
Walk in. Arrive at least 15 minutes before close for service.
Required Documents
Completed application 24 hours in advance. Proof of address: Rent receipt or Utility bill. Proof of income: Paycheck stub. Proof of identification for minors: Birth certificate, Immunization record, Social Security or Medicaid card.
