The client"s income must be below 185% of the federal poverty level, on Fridays must be over 60 years old and have a disability.
Proceso de solicitud
Walk in during business hours. For a first-time visit, arrive Tu - Th from 9 am - 11 am to submit an application. Visits are limited to once per month.
Documentos necesarios
Photo ID and valid food card, or Photo ID and current utility bill, or Photo ID and current lease. To apply for a food card bring the following documents: Photo ID for every adult in the household (can be a work badge, ID card, or any type of identification with a photo); utility bill or rental agreement; pay stubs or DWS myCase printout; and for every child in the household bring birth certificates (or school or immunization record with birth dates)
