Households must make 85% of the Federal Poverty Limit are permitted 2 visits per month.

Walk in or fill out application online.
Required Documents
Utah photo ID card, and proof of income and address; this is either a utility bill, rental contract, or a mortgage statement. Must bring a pay stub for each working member of household and proof of address.
